- Submission Form Requirements

- Reason for Request
Provide a clear and truthful explanation for your request. Inaccurate or misleading information may result in rejection. - Identity Document
Upload a valid original ID (KTP). Photocopies, scanned documents, or unclear images will not be accepted. - Request Letter

- Selfie Verification Please ensure the following when taking a selfie:
- Face, ID (KTP), and request letter are clearly visible
- All information is readable
