- Submission Form Requirements
All information provided in the submission form must be accurate and reflect the actual condition.
- Reason for Request
Provide a clear and truthful explanation for your request. Inaccurate or misleading information may result in rejection. - Identity Document
Upload a valid original ID (KTP). Photocopies, scanned documents, or unclear images will not be accepted. - Request Letter
Users may prepare a request letter independently or use the provided format.
The letter must include:- Full identity details as per KTP
- A clear explanation of the request
- A signature matching the identity document
- Selfie Verification Please ensure the following when taking a selfie:
- Face, ID (KTP), and request letter are clearly visible
- All information is readable
- Adequate lighting is used and the image is not blurry
